Registered Nurse (RN) Case Manager - Home Health | Dallas-Fort Worth

Compensation: $95,000–$110,000 annually Benefits : Excellent Medical, dental, and vision insurance Schedule: Monday–Friday Position Summary We are seeking an experienced Registered Nurse (RN) Case Manager to join our growing team in the Dallas–Fort Worth area . This is an excellent opportunity for an experienced home health RN who is passionate about patient-centered care, clinical excellence, and ensuring patients receive well-coordinated services throughout their care journey. The RN Case Manager plays an important clinical leadership role in coordinating high-quality care across the home health continuum. This position oversees assigned patient cases, ensuring services are delivered in accordance with established Plans of Care, physician orders, agency standards, and regulatory requirements . Serving as a key clinical resource and patient advocate, the RN Case Manager partners closely with nursing, therapy, field clinicians, physicians, and leadership to support continuity of care, appropriate resource utilization, regulatory compliance, and positive patient outcomes. The ideal candidate brings strong home health case management experience, clinical judgment, communication skills, and attention to detail , along with the ability to effectively manage multiple priorities in a collaborative environment. Key Responsibilities Coordinate patient care: Serve as a primary clinical point of contact for assigned patients and coordinate services throughout the home health episode. Review clinical documentation: Collect and review Start of Care (SOC), recertification, evaluations, re‑evaluations, and other clinical reports submitted by field clinicians. Manage Plans of Care: Develop, review, and modify Plans of Care and physician orders to ensure clinical appropriateness, accuracy, and regulatory compliance. Monitor patient outcomes: Track hospitalizations, falls, infections, incident reports, changes in condition, and other clinical events to ensure appropriate follow‑up and reporting. Provide clinical oversight: Support and guide clinical staff to ensure services are delivered in accordance with Plans of Care, physician orders, and agency standards. Support field clinicians: Provide timely clinical guidance, education, and resources to nursing and therapy staff as needed. Advocate for patients: Promote care that is safe, timely, effective, efficient, equitable, and patient‑centered. Collaborate across disciplines: Partner with nursing, therapy, physicians, leadership, and other members of the interdisciplinary care team to address patient needs and support continuity of care. Communicate patient status: Ensure changes in patient condition, clinical concerns, and care needs are communicated to the appropriate internal and external care team members in a timely manner. Participate in case conferences: Contribute to interdisciplinary meetings, case conferences, and clinical care discussions. Coordinate discharge planning: Assess readiness for discharge, ensure treatment goals have been met or appropriately transitioned, provide final education and resources, and coordinate post‑discharge services when appropriate. Support staff development: Participate in clinical education, mentoring, and ongoing development of staff. Drive continuous improvement: Partner with clinical leadership to identify opportunities to strengthen workflows, documentation, care coordination, compliance, and overall clinical operations. Participate in monthly on‑call coverage as assigned. Perform additional duties and responsibilities as assigned by the Director of Nursing (DON), Assistant Director of Nursing (ADON), or Director of Case Management . Required Qualifications & Experience Active and unrestricted Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license . Minimum of two (2) years of RN Case Management experience required. Minimum of one (1) year of Home Health RN Case Management experience required. Strong knowledge of home health clinical operations, care coordination, Plans of Care, and physician orders. Strong clinical assessment, critical‑thinking, organizational, and problem‑solving skills. Excellent verbal and written communication skills with the ability to collaborate effectively with clinical and operational teams. Ability to review and analyze clinical information to support appropriate care decisions and patient outcomes. Experience with patient assessment techniques, including vital signs, physical assessments, phlebotomy, wound VACs, venipuncture, and specimen collection and processing. Experience working with diverse patient populations, including pediatric, geriatric, and/or individuals with developmental disabilities . Ability to prioritize multiple patient cases, clinical needs, and documentation requirements in a fast‑paced environment. Valid Texas Driver’s License. Reliable transportation with current automobile liability insurance. Authorization to work in the United States. This is a primarily in‑office position during normal agency operating hours, Monday through Friday . Occasional patient home visits may be required based on clinical or operational needs and as determined by the Director of Nursing. The RN Case Manager may also participate in a monthly on‑call rotation and be scheduled as part of limited holiday skeleton‑crew coverage; additional compensation will be provided for qualifying patient home visits, on‑call rotation responsibilities, and holiday skeleton‑crew coverage, in accordance with Performance Home Health compensation guidelines.
