Position Job Title: Case Manager (Behavioral Health Technician) – full-time Location: Cholla Health Center, Scottsdale, AZ 85257 Salary Range: $19.47 – $25.26 hourly Job Category: Health Care Responsibilities Assist in identifying, implementing, maintaining, monitoring, and/or modifying behavioral health services on a routine basis. Assist in identifying, finding, and starting/implementing necessary resources that support activities of daily living and address other Social Determinants of Health (SDOH) needs beyond behavioral health services. Coordinate care with the patient, Health Care Decision Maker (HCDM), healthcare providers, family, community resources, and other support systems, including educational, social, judicial, and State agencies. Coordinate care activities to ensure continuity of care between levels of care (e.g., inpatient to outpatient) and across services (e.g., personal care, housing, nursing, and family counseling) and providers. Assist patients in applying for social security benefits. Provide outreach and follow-up after crisis events. Participate in formal and informal case staffing, case conferences, and other meetings with or without the patient/HCDM, DR, or family. Support, collaborate, and manage with assigned clinicians/providers on caseloads and identified needs. Assist in maintaining, monitoring, and modifying covered services as outlined in the patient’s service plan to address identified clinical needs. Perform other activities as needed to address the patient’s treatment needs. Support rescheduling, outreach, and reengagement efforts as needed. Adhere to all rules and regulations regarding patient confidentiality and patient rights per contracts, NOAH policy, and State and Federal regulations. Maintain productivity expectations. Perform other related duties as assigned. Qualifications Knowledge of case management principles and practices. Ability to establish and maintain effective working relationships. Excellent verbal and written communication skills and ability to interact with individuals from diverse backgrounds. Ability to understand and follow written and verbal instructions. Excellent organizational skills and attention to detail. Ability to exercise confidentiality, discretion, and good judgment. Proficient with Microsoft Office Suite or related software. Ability to read, write and speak English; Spanish is preferred. Education and Experience High school diploma or equivalency, plus education in a field related to behavioral health and at least two years of full-time behavioral health work experience. Associate’s degree with at least two years of full-time behavioral health work experience. Bachelor’s or Master’s degree with one year of behavioral health experience, or a Bachelor’s or Master’s degree with a behavioral health focus. Other Requirements New hires are required to pass pre-employment background check and drug testing (effective 11/1/2022). Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.
