Registered Dietitian
The Registered Dietitian provides comprehensive nutrition assessment, Medical Nutrition Therapy (MNT), education, counseling, and nutrition support to patients of Health Care Partners of South Carolina (HCPSC). Working within a Federally Qualified Health Center (FQHC), the Registered Dietitian serves as part of an integrated care team supporting patients with diabetes, hypertension, obesity, cardiovascular disease, renal disease, and other acute and chronic health conditions affected by nutrition.
The Registered Dietitian works collaboratively with medical providers, nursing, behavioral health, pharmacy, and other members of the healthcare team to develop individualized nutrition care plans, improve patient outcomes, and address barriers that may affect a patient's ability to achieve nutrition and health goals. This position also supports preventive health, chronic disease management, quality improvement, and community health initiatives.
Essential Duties and Responsibilities
• Conduct comprehensive nutrition assessments and develop individualized nutrition care plans based on patient diagnoses, health goals, laboratory results, medications, dietary needs, and provider recommendations.
• Provide Medical Nutrition Therapy (MNT) and nutrition counseling within the Registered Dietitian's scope of practice.
• Provide nutrition education and management for patients with diabetes, hypertension, obesity, cardiovascular disease, renal disease, gastrointestinal conditions, and other nutrition-related conditions.
• Educate patients and families regarding meal planning, portion control, food selection, label reading, weight management, and disease-specific nutrition recommendations.
• Establish realistic and measurable nutrition goals and monitor patient progress toward achieving established goals.
• Collaborate with medical providers, nurses, behavioral health professionals, pharmacists, and other members of the healthcare team to support coordinated, patient-centered care.
• Evaluate patient progress and modify nutrition interventions and care plans based on clinical outcomes and individual patient needs.
• Identify social, financial, cultural, and other barriers affecting nutrition and connect patients with available community or organizational resources when appropriate.
• Document nutrition assessments, MNT services, interventions, education, patient goals, and follow-up activities accurately and timely within the Electronic Health Record (EHR).
• Ensure documentation supports applicable billing, coding, payer, and regulatory requirements for nutrition services.
• Support clinical quality measures and organizational initiatives related to diabetes, obesity, cardiovascular health, preventive care, and other nutrition-sensitive conditions.
• Participate in patient education programs, group education sessions, community outreach, and health promotion activities as needed.
• Maintain compliance with HIPAA, HRSA, FQHC requirements, professional standards, and applicable state and federal regulations.
• Participate in staff meetings, quality improvement activities, and interdisciplinary care planning.
• Perform other duties as assigned.
Education & Experience
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR) required.
• Current and unrestricted South Carolina license to practice dietetics required.
• Must maintain all professional credentials, registrations, and licensure required to provide services within the scope of the position.
• Experience providing Medical Nutrition Therapy, nutrition counseling, and chronic disease education in an outpatient, primary care, or community healthcare setting preferred.
• Experience working with patients with diabetes, hypertension, obesity, renal disease, and other chronic health conditions preferred.
• Experience within a Federally Qualified Health Center (FQHC), community health center, or similar healthcare setting preferred.
• Experience using Electronic Health Record (EHR) systems preferred.
Knowledge, Skills, and Abilities
• Strong knowledge of Medical Nutrition Therapy, nutrition assessment, chronic disease management, and evidence-based nutrition practices.
• Knowledge of documentation and regulatory requirements applicable to nutrition services and Medical Nutrition Therapy.
• Ability to develop individualized nutrition care plans based on patients' clinical needs, available resources, culture, and health goals.
• Strong patient education, counseling, communication, and interpersonal skills.
• Ability to collaborate effectively within an integrated, multidisciplinary healthcare team.
• Strong organizational skills with the ability to manage patient appointments, documentation, follow-up, and multiple priorities.
• Proficiency with Electronic Health Record (EHR) systems and healthcare documentation.
• Ability to maintain confidentiality and protect patient information in accordance with HIPAA and organizational policies.
