Requirements:
- MDor DO degree from an accredited medical school.
- Completedresidency in Family or Internal Medicine accredited by the AccreditationCouncil for Graduate Medical Education
- Boardcertified or board eligible in Family or Internal Medicine
- Activeand unrestricted medical license to practice in Arizona.
- CurrentDEA registration for prescribing controlled substances.
- Priorexperience in a family medicine setting preferred.
