The Healthcare Sales Specialist, also referred to as the Outreach Specialist , is responsible for generating qualified referrals and successfully enrolling prospective participants into the PACE program.

This role focuses on grassroots marketing, business-to-consumer sales, community engagement, and relationship development. The ideal candidate is a proactive “hunter” who can identify new referral opportunities, build relationships with community partners, educate prospective participants and referral sources, and consistently achieve monthly referral, home visit, and enrollment goals.

The Outreach Specialist works as part of the Marketing, Outreach, and Enrollment (MOE) team and provides backup support for outreach and enrollment activities as needed.

Key Responsibilities

• Plan and execute community outreach, public relations, and engagement activities to achieve monthly referral, home visit, and enrollment targets.

• Identify new business opportunities through grassroots marketing and community-based prospecting.

• Develop and maintain strong relationships with community leaders, physicians, hospital discharge planners, post-acute facilities, and other referral sources.

• Identify and establish partnerships with community organizations that align with the organization's mission and services.

• Develop and implement annual marketing plans in collaboration with the MOEE Director to achieve enrollment goals.

• Educate referral sources and community members about PACE services and program benefits.

• Follow the MOEE Playbook and comply with applicable rules governing permitted and prohibited marketing practices.

• Identify opportunities to improve outreach processes and enrollment performance.

• Maintain accurate documentation and prepare timely reports using Salesforce, Athena, and Microsoft Office applications.

• Protect confidential company information and the privacy of participants, families, and team members.

• Participate in quality improvement initiatives, required training, staff meetings, and occupational health screenings.

• Communicate regularly with supervisors and team members regarding workload, priorities, and progress toward goals.

• Demonstrate flexibility in work assignments and schedules, including evenings, weekends, and overtime when required.

• Perform additional duties as assigned.

Required Qualifications

• Spanish-speaking proficiency is required.

• Experience in business-to-consumer sales.

• High school diploma or equivalent.

• Experience appropriate to community outreach, field sales, marketing, or participant enrollment.

• Ability to develop new relationships and generate qualified referrals.

• Strong interpersonal, communication, relationship-building, and organizational skills.

• Ability to maintain accurate records and complete required documentation.

• Ability to protect confidential information and follow organizational policies.

Preferred Candidate Profile

The ideal candidate will have:

• Approximately 2–5 years of relevant experience.

• Strong grassroots marketing and community outreach experience.

• A proven ability to prospect for new opportunities and generate referrals.

• A “hunter” mentality, with the motivation to develop new relationships rather than rely solely on existing accounts.

• Experience in healthcare sales, senior services, community engagement, or referral-based business development.

• Experience using Salesforce, Athena, or comparable CRM and documentation systems.

• The ability to work toward measurable referral, home visit, and enrollment targets.

Work Schedule and Environment
