IOP/PHP Care Navigator

St. Louis Behavioral Medicine Institute

Overview At St. Louis Behavioral Medicine Institute (SLBMI), the IOP/PHP Care Coordinator provides administrative support for the IOP/PHP Programs. This role oversees the admission process, clinical assignment, chart audit, and reporting to support adherence to accreditation standards and high-quality care. The Care Coordinator collaborates with the IOP Care Navigator to improve the patient experience and manage admission and discharge processes. This position is primarily housed at the Macklind location with 1-2 days per week at the Chesterfield location. This is an in-person position. Shift: Monday - Friday, day-shift. SLBMI is a freestanding outpatient facility serving local patients and nationally via telehealth. It utilizes a team of 75+ licensed providers and offers specialist behavioral health care for complex conditions. Responsibilities Oversee and improve patient entry, census, and discharge processes to meet/exceed budgeted census expectations; communicate with referral sources before admission and after discharge to build referral relationships to the IOP. Provide weekly reporting to the Director, CEO, and CFO. Assist with initial and ongoing Insurance Authorizations for patients in IOP/PHP as needed. Maintain communication with patients identified as IOP/PHP candidates, including next steps, admission dates, and answers to questions regarding the pre-admission process. Support performance improvement with data tracking and reporting for each IOP/PHP from initial evaluation to discharge, including symptom surveys (pre, mid, discharge, post-treatment) and patient satisfaction surveys. Coordinate with the IOP/PHP treatment teams, attend IOP/PHP Program meetings, and maintain ongoing communication; track attendance via team communication and EHR records. Monitor and assist treatment teams in updating treatment plans during weekly staffing to align with identified compliance standards. Oversee documentation completeness (treatment plans, quality improvement measures, discharge plans, and IOP-related documents); communicate incomplete documentation to the IOP Director and clinical staff. Coordinate discharge planning at admission and throughout treatment, ensuring completion of discharge summaries and transitions at discharge. Serve as a liaison for outpatient providers (internal and external) to communicate and coordinate care at admission, progress updates, and discharge planning; facilitate referrals for aftercare and discharge (e.g., psychiatry, dietitian, or outpatient services). Knowledge of spreadsheets and data manipulation/interpretation. Ability to work independently with effective time management. Customer service orientation. Problem-solving skills.
