You’ll guide teammates, collaborate with management, and consult on treatment plans to support individuals in crisis. Complete initial assessments for new guests. Provide service coordination and support to individuals focusing on the person’s Individualize Support Plan, treatment goals and discharge plans. Assist in the development, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the guest and therapist. Advocate for guests’ rights and preferences. Provide ongoing assessment, problem solving, side by side services, skill training, and environmental adaptations to assist individuals with activities of daily living as needed. Work collaboratively with the clinical team in the on-going assessment of participant’s mental health struggles and responses treatment Facilities Operation and Upkeep Crisis Stabilization Maintain vigilance and be prepared to respond to a resident’s needs and emergencies appropriately by intervening directly and/or contacting indicated supportive staff, agencies and services. Possess the ability to see and hear all crises/emergencies to ensure resident and site safety and security. Work with the guests to develop their crisis stabilization plan, provide scheduled check-ins on stabilization goal progresses.
