Growing together.Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.Schedule: Monday - Friday, 7:30AM - 5:00PM. Supervisor determines schedule within the hours of 7:30AM - 5:00PM.Location: 1580 W Antelope Dr. Suite 200, Layton, UTPrimary Responsibilities:Provide exceptional service to all customersResponsible for adherence management for quality measuresEnsure demographics including insurance information are correct in scheduling and practice management software, along with updates as necessary per Standard WorkSchedule appointments, verifying correct visit type and Primary Care Provider empanelmentWork closely with Providers on care for Medicare population to ensure all Medication, HEDIS measures, and Annual Wellness Exams are completed each yearWork with back-office staff to ensure smooth patient flowResolve service issues and complaints and defer to appropriate management or clinical personnel when necessaryAssist co-workers and team members with duties when requested, to include but not limited to, floating to other areasAnd other duties as assignedWhat are the reasons to consider working for UnitedHealth Group? The hourly pay for this role will range from $18.00 to $32.00 per hour based on full-time employment.
