Position Overview

This position requires an active California CNA license and is essential for providing individualized care for residents in a Skilled Nursing Facility (SNF). The role involves collaboration with nursing staff, residents, families, and various departments within the facility to ensure optimal care delivery.

Shift Information

Shifts include days, nights, weekends, and holidays, with potential to float to other units as needed.

Key Responsibilities

• Receive and give reports on resident conditions, ensuring clear understanding of care plans.
• Conduct rounds on all residents, assessing new residents and those who are ill, have fallen, or suffered injuries.
• Document assessments in compliance with facility policies.
• Notify nursing staff of any changes in resident conditions or needs.
• Assist with direct patient care, ensuring timely response to call lights.
• Foster positive relationships with other departments, colleagues, residents, families, and physicians.
• Perform additional duties as assigned to support overall facility operations.

Qualifications

• Active California CNA license is required.
• Strong communication skills and a commitment to teamwork.
