PRN Home Health Physical Therapist Assistant

HCA Healthcare is seeking a PRN Home Health Physical Therapist Assistant to join our Healthcare & Medical Services team, delivering compassionate, patient-centered care in the home setting. As part of one of the nation’s leading healthcare providers, you will support patients across a variety of diagnoses, helping them regain function, mobility, and independence where they live. Working under the supervision and direction of a licensed Physical Therapist, you will implement established plans of care, provide hands-on therapeutic interventions, and play a key role in helping patients achieve meaningful outcomes.

In this PRN role, you will travel to patients’ homes to conduct visits, carry out therapeutic exercises, gait and balance training, transfer training, and functional mobility activities, and reinforce safe use of assistive devices and equipment. You will carefully observe and measure patient status, documenting progress, tolerance to treatment, and any changes in condition using our electronic medical record system. You will communicate frequently with the supervising Physical Therapist and the wider interdisciplinary team to ensure care is coordinated, timely, and aligned with each patient’s goals and safety needs.

Education and coaching are central to this position. You will instruct patients and caregivers in individualized home exercise programs, safety strategies, fall prevention, energy conservation, and techniques that support continued progress between visits. You will be expected to follow strict infection control protocols, maintain a safe environment in each home, and respond appropriately to changes or emergencies while upholding all HIPAA and regulatory requirements.
