About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of Home Health experience License: Valid state license required Certifications: BLS required Must-Have: - Experience with HomeCare HomeBase - Ability to manage a patient ratio of 5 visits per day - Experience with a variety of diagnoses, including discharges from hospitals, medical offices, and skilled nursing facilities Description: This position requires a Registered Nurse to provide home health services Monday through Friday. The work schedule is from 8:00 AM to 5:00 PM, with no weekends or holidays. The nurse will cover assigned cases in the field, focusing on adult and geriatric patients. Experience with HCHB (HomeCare HomeBase) and home care is necessary. Responsibilities include conducting admissions assessments, managing various medical equipment, and ensuring adherence to infection control protocols. The nurse will handle medication administration, monitor patient conditions, and maintain clinical documentation. Collaboration with interdisciplinary teams and coordination of care services will be essential. The ideal candidate will be well-versed in care management practices and have strong assessment skills. Candidates should be prepared to work with patients requiring a wide range of medical attention, including wound care and symptom management.
