Physical Therapist

Physical Therapist providing in-home, senior living, and virtual care to older adults in West Des Moines, IA. Responsibilities Deliver one-on-one physical therapy in private residences and senior living communities. Perform evaluations, develop personalized care plans, and provide evidence-based treatments. Support patient wellness, mobility, independence, and aging-in-place goals. Document care accurately in electronic medical records and collaborate with interdisciplinary teams. Educate patients, caregivers, and staff on safety and functional maintenance. Incorporate telehealth and virtual care to enhance accessibility and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited Physical Therapy program. Proficiency in computer use and EMR systems. Current CPR certification. Physical Requirements: Ability to lift up to 50 lbs, assist with patient transfers, perform therapeutic activities, and drive to patient locations.
