Enjoy a flexible schedule with primarily daytime business hours, occasional on-call responsibilities, and the chance to make a real difference every day.If you're looking to step away from the fast pace of bedside nursing without giving up the part of nursing you love most—connecting with people and improving lives—we'd love to welcome you to the Venture Forthe team.What You'll Do:Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care.Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.Perform in-home patient assessments, reassessments, and provide intermittent nursing services.Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.Supervise and provide in-service education for staff, conducting competency evaluations.Participate in quality improvement programs and policy reviews.Provide 24-hour on-call nursing coverage on a rotating basis.May assist with PCA Training Program as Needed.Sign-on Bonus up to $10,000Venture Forthe Inc. is an Equal Opportunity Employer and Prohibits Discrimination and Harassment of any kind.
