MDS Coordinator

Riverside Health and Rehabilitation Center is seeking an experienced MDS Coordinator to join our clinical team in Trenton, NJ. The MDS Coordinator will oversee the completion and accuracy of resident assessments while helping ensure regulatory compliance and appropriate reimbursement.

Responsibilities

Coordinate and complete MDS assessments accurately and within required timeframes
Develop and maintain assessment schedules for Medicare, Medicaid, and OBRA residents
Ensure compliance with federal and New Jersey regulations
Lead interdisciplinary care-plan meetings and ensure care plans reflect residents' needs
Review clinical documentation for accuracy, completeness, and reimbursement support
Monitor Medicare eligibility, skilled coverage, and assessment requirements
Apply current PDPM guidelines and identify appropriate clinical categories
Collaborate with nursing, therapy, social services, dietary, and other departments
Educate clinical staff on documentation requirements and MDS-related processes
Monitor quality measures and help identify opportunities for improvement
Participate in audits, surveys, and utilization-review activities

Requirements

Current and valid New Jersey RN license
Prior MDS Coordinator experience is required
Strong knowledge of MDS 3.0, RAI guidelines, PDPM, Medicare, and skilled nursing regulations
Experience with PointClickCare preferred
Strong organizational skills and attention to detail
Ability to manage assessment deadlines and competing priorities
