On-Call Community Child Behavioral Health Case Manager

Merakey is seeking a Case Manager to join the Children’s Behavioral Health Program serving clients in New Castle County, Delaware. The role focuses on coordinating resources, documenting care, and developing family-centered discharge plans, with sessions in homes, schools and community settings.
