JOB SUMMARY: Under the supervision of the Assistant Director, the Family Advocate conducts basic social casework and coordinates related parent involvement services, including assisting youth in setting personal goals around housing, employment, education, and mental health services. In addition, Family Advocates will guide parents in their children's education and development, coordinating health, nutrition, mental health, disability, and parent involvement-related services for families, and being the point person for family case management. JOB SUMMARY: Under the supervision of the Assistant Director, the Family Advocate conducts basic social casework and coordinates related parent involvement services, including assisting youth in setting personal goals around housing, employment, education, and mental health services. In addition, Family Advocates will guide parents in their children's education and development, coordinating health, nutrition, mental health, disability, and parent involvement-related services for families, and being the point person for family case management. ESSENTIAL JOB DUTIES:
- Conducts family assessment of each enrolled family by identifying family strengths, interests, and needs.
- Support clients as they navigate the housing stability process including, support with obtaining needed documents for establishing eligibility, housing search and landlord engagement, referrals to community resources that match client goals, increase in income through employment and benefits
- Utilize a trauma-informed/harm reduction/housing first and solution-oriented model when working with clients.
- Respond to urgent requests for assistance from clients or the property manager as needed
- Complete thorough and objective case notes and service entry data for CLARITY in a timely manner in accordance with SHP policies and procedures.
- Complete all forms/reports required for Supportive Housing Programs and submit by the established deadlines.
- Obtain and retain all required documentation (e.g., leases, income verification) as required by the program.
- Participate in case conferences, meetings with collaborative agencies, and other community meetings as directed and be available for impromptu conference calls as needed
- Orient new intakes to the program, including completing documentation.
- Develop a self-sufficiency plan with each resident that describes objectives, action steps, timeframes, and supportive services necessary to achieve the objectives.
- Routinely conduct apartment checks, common area checks, and complete work orders to maintain apartment and facility safety.
- Co-Facilitate and assist with groups such as Life Skills Workshops.
- Provide crisis intervention to residents and develop safety plans to assist residents with making safe choices.
- Actively participate in mandatory trainings, in-services, and meetings.
- Maintain a healthy and safe therapeutic environment for all residents.
- Assist with the operations of the lobby when necessary.
- Assist during emergency situations. Direct emergency response personnel, residents, and staff. Assist with evacuating the building if necessary.
- Participate in the Continuous Quality Improvement (CQI) of the agency.
- Communicate through fax, telephone, and e-mail with other service providers when appropriate.
- Maintain confidentiality as required by St. Anne's, Licensing, HIPAA, and other regulatory agencies.
- Other duties as assigned
- Bachelor's Degree in a Behavioral Science and a minimum of two years of full-time experience working with transition-aged youth, or a Master's Degree in a Behavioral Science or Human Service based on current managing contract guidelines.
- Good oral, written, and organizational skills.
- Must possess and maintain a valid California Driver's License and a driving record acceptable to the agency's insurance carrier.
- Physical ability to perform Pro-ACT, administer first aid/CPR, and operate an Agency motor vehicle.
- Ability to independently use a computer and the ability to learn necessary computer programs.
- Ability to receive and implement constructive feedback.

As such, employees are expected to maintain composure during all interactions and crisis situations, implement a self-care plan, and utilize crisis communication to deescalate client crisis and prevent any serious bodily injury or harm.
