On-Site Center Nurse: Chronic Care & Patient Wellness

Oak Street Health is seeking a Center Nurse in Indianapolis to join a value-based care team focused on outcomes and prevention. You will perform assessments, manage chronic conditions, administer vaccines, and educate patients and caregivers in person, by phone, or in group settings.
