Supervise and support Care Coordinators, including assigning tasks, monitoring workloads, and ensuring compliance with policies and procedures. Oversee patient intake and care coordination processes, ensuring continuity, quality of care, and timely service delivery. Receive and review referrals from healthcare providers, ensuring documentation is complete and accurate. Manage caregiver scheduling and staffing coverage based on authorized hours, caregiver skills, and client preferences. Serve as the primary point of contact for patients, families, caregivers, and healthcare teams regarding care coordination needs. Communicate patient updates, care plans, and scheduling changes while maintaining accurate and confidential records. Participate in care conferences, resolve service barriers, and support billing and service verification processes. Mentor and provide guidance to care coordination staff on complex cases and day-to-day operations. Educate patients and families on available services, community resources, and care expectations. Support quality improvement initiatives, data collection, compliance efforts, and participate in on-call rotation as needed. Perform other duties as assigned.
