Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Address, identify, and reassess cost-efficient and appropriate levels of care
• Coordinate non-medical support to ensure treatment compliance
• Coordinate care transitions and engage specialty resources as needed
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as a patient advocate, protecting privacy and confidentiality

Requirements of the RN - Case Management

• RN KY license
• Recent experience in Home Health with OASIS and HCHB
• Associates or Bachelor's Degree in Nursing or related field
• Ability to analyze complex medical information
