You will use **Natural Language Processing (NLP)** screening of radiology reports to identify at-risk patients, drive structured workflows, and deliver guideline-aligned clinical care.Time Allocation* **50% Patient Navigation & Care Coordination** (Chart reviews, registry tracking, scheduling, patient outreach).* **50% Direct Clinical Care** (In-person outpatient visits and longitudinal management).Key Responsibilities*Patient Navigation & Operations (~50%)** **Screen & Review:** Evaluate NLP-flagged cases via structured EHR chart reviews to confirm program eligibility.* **Outreach:** Engage primary care teams and contact patients via phone, letters, and MyChart.* **Coordinate Care:** Facilitate diagnostic pathways (PFTs, sputum cultures, labs, imaging) and ensure timely pulmonology scheduling.* **Educate:** Provide vital patient education on care plans, timelines, and symptom escalation.* **Track Data:** Document activities in the EHR and maintain accurate program tracking in REDCap.*Clinical Care (~50%)** **Outpatient Visits:** Conduct clinical evaluations and longitudinal management for bronchiectasis, lung nodules, COPD/emphysema, and ILA/ILD.* **Assessments:** Perform risk stratification, symptom reviews, and develop evidence-based care plans under physician supervision.* **Collaborate:** Support multidisciplinary case reviews and participate in Lung Health Team meetings.Qualifications*Required:** Licensed Nurse Practitioner or Physician Assistant eligible to practice in MA.* Strong organizational skills with comfort balancing clinical care and outreach.* Excellent communication skills with patients and multidisciplinary stakeholders.* Proficiency with EHR workflows.*Preferred:** Outpatient pulmonary clinical experience.* Experience with patient navigation, population health, or structured care pathways.* Familiarity with REDCap or similar registry databases.* Interest in technology-enabled care models.Professional Development & Perks* **Mentorship:** Work closely with program leadership within a heavily supported, grant-funded initiative.* **Scholarly Output:** Opportunities for abstract/manuscript authorship and presentations at national meetings.* **Future Growth:** Gain direct involvement in upcoming clinical trials and research initiatives.*Beth Israel Lahey Health, including all of its employing entities, are a purpose-driven, values-based organization that unites our workforce of people who provide exceptional health care to everyone we serve.**All of the employing entities of Beth Israel Lahey Health maintain policies of equal employment opportunity and non-discrimination in employment.
