Hospice RN Case Manager - Adrian / Tecumseh / Jackson

Registered Nurse - HOSPICE

Location: Jackson, MI. Main Service Area: Adrian, Tecumseh, and surrounding areas. This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.

Hours: Monday to Friday, 8 a.m. to 5 p.m. Weekend On-Call Rotation.

Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building rapport with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Required Qualification

• Graduate of an accredited nursing school with an Associate or Bachelor Degree in Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Hospice or home health experience is a plus

• Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Ability to handle multiple priorities, documentation requirements and deadlines

Pay Range $75,000 $82,000 USD
