Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
- Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
- Document patient progress, treatment sessions, and discharge planning in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
- Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current and valid Occupational Therapist (OT) licensure in the state of New York
- National Board for Certification in Occupational Therapy (NBCOT) certification
- Experience working in a skilled nursing facility (SNF) setting preferred
Preferred Experience / Skills
- Prior experience in a SNF or post-acute care environment
- Familiarity with Medicare documentation and compliance requirements in a SNF setting
