Home Dialysis Nurse Educator & Case Manager

Fresenius Medical Care

Fresenius Medical Care seeks an experienced RN to serve as an onsite Patient Educator and Case Manager for home dialysis therapies. You will train patients and families, coordinate with the clinic team, and support quality care through the Home Therapies program. The role includes home visits with another staff member, ensuring safety, educating about ESRD and CKD options, and participating in CQI activities.
