Plan Specialist - Full Time - Flexible

JOB TITLE : Plan Specialist - SEMP

CLASSIFICATION: Full Time, Non-exempt

PROGRAM/DEPARTMENT: Vocational Services

IMMEDIATE SUPERVISOR: Director of Vocational Services

Supports adults with disabilities in achieving their vocational goals by developing person centered plans along with coordinating, implementing and facilitating activities to include, managing the development, maintenance and communication of mandatory tracking and reporting of progress to goals. Serves as a liaison to community partnerships. Work with the ETP supervisor and attend quarterly meetings, submit monthly summaries and prepare Discovery paperwork for individuals that may be appropriate candidates for the program. Engage with community partners regarding service, process, progress and feedback. Provide job coaching supports to for both OPWDD and ACCES VR participants on your case load.

QUALIFICATIONS:

Minimum Education, Training & Experience:
Associates’ Degree in human services, education, psychology or related field plus one year of experience of working with adults with developmental and/or cognitive disabilities

Driver’s License with an acceptable driving record and dependable transportation

Preferred Education, Training & Experience:

Bachelor's degree preferred

Ability to be positive and outgoing and demonstrate strong interpersonal skills;

Ability to work flexible hours, including occasional nights and weekends

Ability to assist the people we support to pursue and obtain their personal goals;

Ability to effectively communicate orally, in writing, and on the telephone;

Ability to work effectively without direct supervision to ensure completion of mandatory daily and monthly documentation;

Ability to work cooperatively with all departments, levels of staff, and outside contacts;

Ability to travel to sites in Cattaraugus County for community-based work experiences or employment

opportunities.

Ability to effectively balance multiple tasks and ever-changing priorities.

Ability to understand, communicate and enforce regulations governing supported employment services

Ability to learn and use online and virtual training formats for group activities.

SALARY:

$19-23per hour based on experience
