Job Summary
Case Management — Emeryville and San Francisco, California. This position supports complex cases across the healthcare system and serves patients in the community through primarily virtual services. The role includes care coordination, utilization review, discharge planning, and collaboration with interdisciplinary teams.
Job Details
- Required Qualifications: California Registered Nurse license; Basic Life Support (BLS) certification; at least 2 years of experience; inpatient case management experience; community hospital experience; travel experience; experience supporting adult and geriatric patients.
- Preferred Qualifications: Previous charge experience; Level I or Level II trauma experience.
- Schedule: Five 8-hour shifts per week, 8:00 AM–4:30 PM, for a minimum of 40 hours per week. The schedule includes two onsite days and three remote days weekly, with onsite work in Emeryville and San Francisco as needed.
- Additional Information: Responsibilities include care coordination, admission and concurrent review, continued stay review, discharge planning, disease management, medical necessity review, prior authorization, retrospective review, needs assessment, durable medical equipment coordination, and utilization of established criteria. The position supports complex cases, telephonic services, interdisciplinary collaboration, and patients receiving care in the community.
