Hospice RN Case Manager- NORTH Brooklyn NY (15K Sign on Bonus)

Hospice RN Case Manager – with $15,000 New Hire Bonus and Pension Salary: $101,000 - $119,000 per year Job Details North Brooklyn, NY. Full‑time. Monday‑Friday 8:30 AM – 5 PM. 4 hospice visits per day. Home charting in afternoon. Territory: small area (Bedford‑Stuyvesant, Bushwick, Greenpoint, Williamsburg). Assignment to 3 zones only. Work primarily in home and facility settings. Responsibilities Provide skilled nursing care to terminally ill patients, supporting patients and families through the end‑of‑life process. Create and follow plan of care to maximize comfort and quality of life. Assess and develop “Goals of Care” with patients and families. Follow infection control standards. Educate patients and families about disease progression, symptom management, and medication administration. Collaborate with physicians for medication titration and update individualized care plans. Work with interdisciplinary team. Recommend medical equipment and supplies, and maintain inventory. Act as liaison for GIP/IPU patients in crisis on the hospice inpatient unit when needed. Maintain accurate and timely clinical documentation. Qualifications RN with active NY State RN license in good standing. At least 1‑2 years of hospice or palliative care experience (or Med‑Surg/Oncology). BSN preferred. Must have a car. Benefits $15,000 sign‑on bonus (depending on position). 4 weeks vacation. 8 company‑paid holidays. Employer‑paid pension 100% at 5.75% salary per year. Vesting at 5‑year anniversary. 403(b) retirement plan thru Lincoln Financial. Medical, dental, vision coverage starting 60 days after start date. Medical coverage thru Cigna. Dental coverage thru Cigna PPO or Aetna DMO. Vision coverage thru United Healthcare. Free online RN to BSN and MSN degree programs. Dependent tuition reimbursement for clinical staff.
