Licensed Vocational Nurse (LVN) – Skilled Nursing Facility

Job Type: Full-time / Part-time
Location: California
Pay: $32-40 per hour, depending on experience
Shift: Evening shift available

We are seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our skilled nursing team at Community Extended Care Hospital of Montclair . The LVN will provide direct nursing care to residents while working closely with Certified Nursing Assistants (CNAs), Registered Nurses (RNs), physicians, and other members of the interdisciplinary team.

The ideal candidate will have strong clinical skills, excellent communication, and a commitment to providing safe, respectful, and resident-centered care.

• Provide nursing care to residents according to physician orders and individualized care plans.

• Administer medications and treatments safely and accurately.

• Monitor and document residents' condition, vital signs, and response to treatment.

• Perform nursing assessments and promptly report changes in residents' condition.

• Provide wound care and skin assessments as ordered.

• Manage and care for residents with feeding tubes, catheters, ostomies, and other medical needs.

• Assist with diabetes management, including blood glucose monitoring and insulin administration as ordered.

• Perform and document treatments and procedures according to facility policies.

• Communicate effectively with physicians, RNs, CNAs, residents, and families.

• Supervise and provide direction to CNAs and other assigned staff.

• Participate in care plan meetings and contribute to resident care planning.

• Maintain accurate and timely medical records and documentation.

• Follow infection-control, safety, HIPAA, and facility policies.

• Respond appropriately to changes in resident condition and emergencies.

• Promote residents' dignity, privacy, independence, and quality of life.

• Current and active California LVN license .

• Current BLS certification .

• Graduate of an accredited Vocational Nursing program.

• Previous SNF, skilled nursing, long-term care, rehabilitation, or geriatric nursing experience preferred.

• Strong clinical assessment and documentation skills.

• Ability to work effectively as part of an interdisciplinary team.

• Excellent communication and organizational skills.

• Reliable, compassionate, and professional approach to resident care.

• Preferred: Korean Speaking
