Physical Therapy Assistant (PTA)

- Role: Physical Therapy Assistant (PTA)
- Schedule: Full-time Monday-Friday + occasional weekends (as needed)
- Location: Field Based
- Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year
- Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality.
- Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
- Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
- Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
- Deliver skilled PTA visits in the home under the supervising PT's plan of care.
- Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
- Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.
- Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.
- Coach patients and caregivers with practical home programs that actually get done.
- Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.
- Dedicated clinician: home health experience strongly preferred
- High ownership: you treat your caseload like your own panel and your pod like your own business.

- Tech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better
- Trust-builder: Patients, families, and referral partners believe you because you do what you say.
- Active Michigan PTA license (in good standing)
- Reliable transportation, valid driver's license, auto insurance
- Home health experience strongly preferred
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PTA scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
- Full-time target: ~30 productivity points/week ; clinicians earn more for exceeding it.
- Guaranteed floor: ~24 points/week for the first two months - if your caseload hasn't filled, you're paid the floor.
- $10k sign-on bonus paid after first 90 days (2-year clawback)
