About the Role:
Talin Home Healthcare LLC is looking for a compassionate and dedicated Home Health Nurse to serve patients in the Culpepper area. Responsibilities:

• Provide skilled nursing care to home health patients in the Culpepper service area
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor and document patient progress, vital signs, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on disease management, medications, and home safety
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Active RN license in the state of Virginia
• Previous home health, community health, or clinical nursing experience preferred
• Strong assessment, critical thinking, and clinical decision-making skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel within the Culpepper area
• Ability to work independently and manage a patient caseload efficiently
• Compassionate, patient-centered approach to care
• Must be willing to work at least 2 days a week with 1 of those days be either Monday or Tuesday.
