MDS Coordinator (RN)

Palm Gardens Center for Nursing & Rehabilitation

Salary: $100,000–$130,000 per year

Palm Gardens Center for Nursing & Rehabilitation is seeking an experienced MDS Coordinator (RN) to join our team.

We are looking for a knowledgeable, organized, and motivated MDS professional who understands the importance of accurate assessments, strong clinical documentation, compliance, and reimbursement.

Responsibilities

• Coordinate and complete MDS assessments accurately and timely

• Manage the MDS/RAI process and assessment schedule

• Ensure compliance with CMS, Medicare, Medicaid, and regulatory requirements

• Review clinical documentation for accuracy and completeness

• Work closely with nursing, therapy, social services, and the interdisciplinary team

• Monitor PDPM documentation and reimbursement

• Identify documentation opportunities and ensure appropriate follow-up

• Participate in care planning and interdisciplinary meetings

Qualifications

• Active RN license

• MDS experience in a skilled nursing facility required

• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and Medicare

• Strong organizational and communication skills

• Excellent attention to detail
