Crisis Care Coordinator – REACH Youth Crisis Team (Milwaukee, WI)Lutheran Social Services of WI & Upper MIJoin a mission-driven team supporting youth in psychiatric crisis through the REACH program—Reaching, Engaging, and Assisting Children & Families. As a Crisis Care Coordinator, you'll provide trauma-informed, person-centered care coordination to help prevent out-of-home placements and connect youth with vital community resources.Key Responsibilities:Conduct assessments and develop individualized service plansFacilitate care coordination and crisis interventionCollaborate with families, providers, and service teamsParticipate in after-hours on-call rotationWhat We Offer:Full-time, benefits-eligible role with flexible schedulingPublic Service Loan Forgiveness (PSLF) eligibilityMedical/Dental/Vision + 403B retirement planPaid time off, holidays, mileage reimbursementOngoing training and professional developmentQualifications:Bachelor's degree in human services (e.g., Social Work, Psychology, Counseling)Minimum 1 year of mental health service experienceValid driver's license and reliable transportationMake a meaningful impact in the lives of youth and families.
