Responsibilities
- Conduct initial PT evaluations and establish individualized treatment plans for residents
- Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional independence
- Reassess patient progress regularly and update treatment plans accordingly
- Document sessions clearly and accurately in the patient record; communicate progress to the care team
- Collaborate with nursing, therapy aides, and other disciplines to coordinate patient care and discharge planning
- Ensure safe patient handling, transfer techniques, and equipment use in compliance with facility and regulatory standards

Requirements
- Active physical therapy license in Washington state or eligibility to obtain licensure in Washington
- Ability to work effectively in a skilled nursing facility setting
- Strong communication and documentation skills
- Proficiency with electronic health records and standard rehabilitation documentation
- Availability to fulfill a 13-week travel assignment starting 09/07/2026
