Roles & Responsibilities
- Perform initial and ongoing assessments of patient\'s condition
- Establish, revise, and evaluate a plan of care appropriate to identified problems, involving the patient/family
- Perform therapy interventions using standard physical therapy techniques and skills appropriate to patient condition
- Oversee activities of physical therapy assistants, students, and other support personnel
- Work some holidays and one weekend per month as needed
Experience Required
- 2 years of relevant experience
- SNF (Skilled Nursing Facility) experience required
Skills & Certifications
- Physical therapy assessment and treatment planning
- Standard PT techniques and interventions
- Supervision of PT assistants and support staff
Eligibilities & qualifications
- Patient/family care coordination
