Medical Social Worker (Hourly)

Role: The Care Manager (CM) oversees a small caseload of private pay clients, providing comprehensive coordination of services, ongoing monitoring, and support within clients's homes. This includes clients in Independent Living, Assisted Living, or Nursing Facilities. The CM develops strong relationships with community and professional organizations to generate referrals and collaborates with healthcare providers, attorneys, and other geriatric professionals. Responsibilities Conduct in-depth client assessments covering medical, psychosocial, environmental, financial, legal, and family aspects. Perform fall risk, mental status, and depression screenings. Develop and implement personalized care plans ensuring client safety and needs are met. Coordinate services and collaborate with clients, families, medical staff, and community resources. Perform regular home visits to monitor client status and environment. Provide emotional support, counseling, and crisis intervention as needed. Assist in business development by engaging with referral sources. Maintain detailed documentation for billing and care tracking. Qualifications & Skills Licensed Social Worker with 7-10 years of geriatrics experience. Ability to work independently with minimal supervision. Valid driver's license and reliable personal vehicle. Proficiency in crisis management, problem-solving, and critical thinking. Strong interpersonal and communication skills.
