Community Health Worker: Outreach & Care Navigator

Indiana University Health

Indiana University Health is seeking a compassionate Community Health Worker to engage patients and families, identify barriers to care, and connect them with essential services. You will support care coordination, conduct SDOH screenings, and provide education on maternal and infant health.
