Social Worker Position

Sanford Health & Rehabilitation located in beautiful Sanford, North Carolina is seeking an experienced Social Worker to join our fantastic team of caregivers! You will work alongside fellow caregivers to provide high-quality patient care.

Responsibilities include:

• Assess nature of patients' situations by interviewing and reviewing personal history
• Develop and execute individual treatment plans/care plans
• Determine appropriate milestones to gauge patient progress
• Communicate patient updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs as needed
• Maintain accurate documentation

Qualifications:

• Previous experience in social work, with a focus on short term and long-term care patients
• Compassionate and caring demeanor
• Ability to build rapport with staff, patients, and their loved ones
• Strong leadership qualities
• Excellent written and verbal communication skills
