We are seeking a compassionate and skilled Home Health Aide (HHA) to join our team at Constant Care Health Services LLC in Bronx, New York, USA. As a member of our team, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes. Responsibilities:
- Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
- Assist with mobility and transfer patients, as needed
- Administer medication and maintain accurate records
- Monitor and report any changes in patient condition to the Nurse, Physician, and family members
- Participate in care planning and development of individualized care plans
- Maintain a clean, safe, and comfortable environment for patients
- Collaborate with the healthcare team to ensure continuity of care

Requirements:
- High school diploma or equivalent required
- Completion of a state-approved Home Health Aide training program
- Current certification as a Home Health Aide
- Ability to lift, transfer, and reposition patients as needed
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
