SLP will be the only SLP in this department. Current caseload is 9 patients with various diagnoses, including CVA, dementia, CHF, COPD, DM2, dysphagia, aphasia, and other cognitive impairment is preferred. We do have an an electronic medical record, but use a SOAP note format when appropriate for documentation. This facility has a large population with cognitive impairment. An ideal candidate would have special training or certification in working with patients with dementia. This position is shared between Ellen Sagar Nursing Center, and Woodruff Manor. The therapists who have been in these facilities have been at Ellen Sagar 3 days, and Woodruff Manor 2 days, weekly.
