Registered Nurse Diabetes Educator

Valley View's Cardiopulmonary Rehab Department is seeking a Registered Nurse Diabetes Educator. This position is responsible for providing Diabetes assessment, education, counseling and self-management training for outpatients for individuals and groups, and diabetes education for inpatients as well. This role also assists as needed with general Cardiopulmonary Rehab diabetes counseling, education, operations and marketing duties.

This position is 24 hours a week, part time for Diabetes education and pump education!

PRIMARY DUTIES AND RESPONSIBILITIES:

• Educates and counsels on health-related topics such as diet/nutrition, exercise, stress and chronic disease management.
• Assesses the nutritional status of patients using parameters such as anthropometric measurements, visual/physical assessment and interpretation of lab values with documentation in the medical record and/or patient forms.
• Assesses and creates care plans for patients with diabetes and obesity, including but not limited to inpatient diabetes and cardiac interventions, diabetes self-management education and support classes.
• Assists in maintaining programming accreditation [ex: Diabetes Self-Management Education and Support (DSME/S) and Diabetes Prevention Program] including required trainings and education requirements.
• Participates and assists in planning of community relations events, health fairs and presentations.
• Maintains a working knowledge of all program-related core competencies.

QUALIFICATIONS:

• Certification or Licensure: Registered Nurse, Registered Dietician or Pharmacist.
• Certified Diabetes Educator (CDCES), or Board Certified in Advanced Diabetes Management (BC-ADM) preferred
• Pump Trainer Certification or willingness to obtain
• Education: Bachelor's degree required.
• Experience: At least 1 year working with a diabetic patient population.
• Skills: Good computer skills; excellent customer service and communication skills. Bilingual (English/Spanish) preferred.

SALARY:

• Entry salary dependent upon education, skill set, and experience.
• Salary range: $37.50-$56.23 / hour
