About the Position

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Help patients improve strength, balance, and functional mobility
• Monitor and document patient progress
• Work closely with therapy and nursing team

Requirements

• Active CA PT License
• BLS Certification
• SNF/LTC experience preferred
