Your typical day will include traveling to patient residences, performing comprehensive assessments, developing and updating individualized care plans, administering medications and treatments, and teaching patients and caregivers how to manage health conditions safely at home.You will coordinate closely with physicians and an interdisciplinary team to ensure continuity of care, timely communication of changes in patient status, and appropriate use of supportive services. Clinical responsibilities may include wound care, monitoring vital signs and lab results, managing chronic disease symptoms, overseeing post-operative recovery, and evaluating the effectiveness of interventions. You will use an electronic medical record system and mobile technology to document visits and communicate with the care team while in the field.Patient caseloads are structured to allow for meaningful, unhurried interactions, focused teaching, and thorough assessments. You will have the opportunity to build long-term relationships with patients and families, seeing the impact of your work as patients gain independence and avoid unnecessary hospitalizations. Schedules typically include daytime shifts with flexibility needed to meet patient needs, occasional weekend or holiday rotation, and travel within a defined territory in Rankin County.This role offers the chance to practice true holistic nursing-assessing the home environment, understanding social and community factors, and tailoring care to each patient's unique situation.
