Job Description Key Requirements

• Active CA Social Worker license/certification (depending on facility requirement)

• Previous hospital or clinical social work experience preferred

• Strong communication and patient advocacy skills

• Experience with discharge planning, psychosocial assessments, and care coordination

• Ability to work in a fast-paced healthcare environment

Key Responsibilities

• Conduct psychosocial assessments for patients and families

• Assist with discharge planning and community resource coordination

• Provide emotional support, counseling, and crisis intervention

• Collaborate with physicians, nurses, and case management teams

• Help patients navigate insurance, placement, and post-discharge care needs
