Salary: $45,000 - $50,000

RESPONSIBILITIES

Recruitment/Assessment/Care Coordination

CMA will perform outreach/recruiting within the community,informing perspective clients ofCM services and scheduling assessments for eligibility.

CMA will schedule and completean InitialScreening appointment using ashortscreener toassesseligibility for CM services and educate onPrEP.

CMA will gather eligibility documents and upload/update them ineCWas needed.

Referrals will be made for CM Intake appointments, other FMCS services or other communityorganizationsto address client needs.

HIV testing and Educationwill be provided tostatus neutral clients.

ProvidePrEPeducation and referrals.

Office Hours:Monday-Friday,8:30am 5:30pm

Qualifications:Associates/bachelors degree in healthor human services related field preferred. High School diploma or GEDrequired. A minimum of 2 years of experience working with persons living with HIV or athigh riskof HIV acquisition preferred.HIV Testing backgroundpreferred.
