Social Worker

Heartland Hospice

Heartland Hospice, part of ProMedica, seeks a compassionate Social Worker MSW to provide psychosocial support to patients with life-limiting illnesses and their families. In this role, you will complete psychosocial assessments, develop and update care plans, provide counseling, crisis intervention, and grief support, and coordinate community resources. You will collaborate closely with nurses, physicians, chaplains, and aides to ensure patient comfort, dignity, and respect. This position offers the opportunity to build strong clinical, advocacy, and communication skills in a mission-driven, interdisciplinary environment.
