Work Environment\nThis position offers a fully remote work setting, allowing for greater flexibility and work-life balance. As a Licensed Clinical Social Worker at Mind Care Therapy Solutions, you will engage with clients using telehealth technologies and digital communication methods. The typical working hours range from Monday to Friday, between 8:00 AM and 4:30 PM, but we also require flexibility to accommodate client appointments outside these hours, including occasional evenings and weekends, depending on individual client needs.
Key Responsibilities\n- \n
- Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of clients. \n
- Develop and implement individualized care plans with measurable goals, involving clients and their families in the process. \n
- Provide counseling and emotional support, addressing life transitions, challenges, and promoting coping strategies. \n
- Advocate for client access to essential resources and community support services, ensuring their rights and needs are met. \n
- Collaborate with a diverse team of healthcare professionals, including nurses and caregivers, to coordinate care effectively. \n
- Regularly monitor clients’ well-being through virtual visits, making adjustments to their care plans as necessary. \n
- Facilitate family communication and conflict resolution to ensure a supportive caregiving environment. \n
- Maintain thorough and organized documentation of client interactions, assessments, care plans, and progress notes. \n
- Stay informed regarding current trends, regulations, and best practices in social work, ensuring ethical compliance. \n
Your primary focus will be on neurodiverse adults, seniors, and individuals dealing with chronic illnesses and disabilities. Qualifications\n- \n
- A Master's degree in Social Work from an accredited institution. \n
- Current and active state licensure as a Licensed Clinical Social Worker (LCSW) in California. \n
- Minimum of one year of directly relevant experience in a clinical or community-based setting. \n
- Strong understanding of social work principles, interventions, and available community resources. \n
- Demonstrated counseling skills and the ability to engage empathetically with clients and families. \n
- Effective communication and interpersonal abilities to collaborate within interdisciplinary teams. \n
- Proficiency in electronic health record (EHR) systems and telehealth applications. \n

- Experience working with senior populations or individuals with chronic health conditions. \n
- Familiarity with telehealth practices and online counseling methodologies. \n
- Two years or more in a social work role within a community health or rehabilitation setting will be considered an asset. \n
The salary for this position ranges from USD 80,000 to 100,000 per year, commensurate with experience and qualifications.
