Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1+ years of experience required, Long Term Care or Rehabilitation experience required License: Active occupational therapy license required Must-Have: - 1+ years of occupational therapy experience - Long Term Care or Rehabilitation experience - Active occupational therapy license - New graduates considered with Level II experience Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients per regulatory and clinical practice requirements. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluations and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Participates in facility and company-required training, in-services, and conferences. Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support occupational therapy assistants, rehabilitation aides, and/or students. Adheres to professional practice standards and ethics during occupational therapy practice.
