Primary Care NP/PA In-Home Health Assessments West Virginia

We are seeking Nurse Practitioners and Physician Assistants for flexible locum tenens opportunities conducting in-home health assessments throughout West Virginia. This position offers flexible scheduling with both short-term and long-term assignment options available.

Position Details

• Specialty: Primary Care / In-Home Health Assessments

• Position Type: Locum Tenens / 1099

• Start Date: As soon as credentialed

• Assignment Options: Full-time, Part-time, PRN, Contract, Temporary, Seasonal

• Flexible scheduling available

• No nights or weekend requirements

Schedule

• Flexible scheduling Monday Sunday

• Daytime hours between 7a 7p

• Providers manage their own schedules within available coverage needs

Practice Setting

• Home Health

• In-home evaluations for Medicare Advantage, Medicaid, and commercial health plan members

• Mobile community-based care model

• mySignify EMR platform

Clinical Responsibilities

Providers will conduct pre-scheduled in-home health evaluations, including:

• Medication reconciliation and medical history review

• Comprehensive health assessments and physical evaluations

• Documentation utilizing patient-specific pre-populated forms

This role does not include:

• Prescribing medications

• Ordering labs or diagnostic testing

• Altering current treatment plans

Patient Volume

• Average 6 9 home visits daily

Required Qualifications

• Active NP or PA license required

• Board certification required

• Master s degree required

• Experience backgrounds accepted include:
