Responsibilities

• Provide direct nursing care to residents within the LPN scope of practice in a long-term care setting.
• Monitor and document vital signs, assess patient status, and communicate changes to the supervising RN.
• Administer prescribed medications and therapies in accordance with facility policies and state regulations.
• Assist residents with activities of daily living, provide mobility support, and promote safety and comfort.
• Participate in care planning and collaborate with the interdisciplinary team to update and implement plans of care.
• Maintain accurate nursing records, respond promptly to resident needs, and uphold infection control and safety standards.
