Registered Nurse (RN) - Case Management

Short Term Acute Care Inpatient Medical Social Worker (MSW)

Schedule: Weekend coverage required

Requirements:

• MSW certification required
• Minimum of 1 year of experience
• Compact state license must be in hand at submission
• Experience with Epic charting system preferred

Roles & Responsibilities:

• Conduct discharge planning and care coordination for patients.
• Provide patient and caregiver education and maintain documentation of teaching.
• Make referrals to rehabilitation and long-term care facilities.
• Assess and document knowledge regarding protective services, suicide/self-harm, and substance use/treatment options.
• Participate in care conferences and collaborate with interdisciplinary teams.
• Document care delivery, plans, and goals effectively.

Compliance / Notes:

• 40 hours of orientation required
• Must commit to working 3 of 5 specified holidays
• All time-off requests must be submitted upfront; late requests may not be granted.
• Candidates should not contact facility representatives directly without prior arrangement.
