Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment sessions, and discharge planning in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
• Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities

Required Qualifications

• Graduate of an accredited Occupational Therapy program
• Current and valid Occupational Therapist (OT) licensure in the state of New York
• National Board for Certification in Occupational Therapy (NBCOT) certification
• Experience working in a skilled nursing facility (SNF) setting preferred

Preferred Experience / Skills

• Prior experience in a SNF or post-acute care environment
• Familiarity with Medicare documentation and compliance requirements in a SNF setting
